Appointment
  • Please complete and submit the vaccine provisional application form.
    Our clinic will contact you by phone or email within 24 hours (excluding Saturday, Sunday, and holidays) to confirm your appointment. *If you wish to receive an English-language certificate, please select a date 5 days or more after the date of application as the preferred date of visit.
  • · Please download the medical questionnaire for the vaccine you wish to receive below and fill out the form.

● Things to bring with you: medical questionnaire form, mother-child handbook, any certificates of vaccination, and vaccine notebook (if you have one)
  • · You cannot receive a vaccination if you have a fever of 37.5°C or higher or are not feeling well.
  • · If you have received any vaccinations from other clinics or hospitals, please pay attention to the vaccination intervals.
After vaccination
  • · Please monitor your condition for changes for 15 to 30 minutes after vaccination.
  • · If you are scheduled to receive another vaccination, please confirm the vaccination plan (type of vaccine and schedule) before going home.
  • · Avoid strenuous exercise on the day of vaccination.
  • · You can take a shower or bath, but do not take a long bath.
  • · Some side effects, such as redness or pain at the injection site, may occur within a week, but in most cases, they will resolve spontaneously within a few days. Contact the clinic if you have any symptoms that interfere with daily activities or any symptoms of concern.

  • ● For the initial vaccination, you need to apply in advance using the vaccine provisional application form. Please complete and submit the vaccine provisional application form.
  • ● Vaccinations are only given by appointment because it is generally necessary to obtain the vaccines in advance. If you wish to receive a vaccination on the day of your visit, please contact the clinic in advance. *We do not make appointments for the influenza vaccine.
  • ● If you wish to receive an English-language certificate, please check with the clinic in advance to see whether that can be accommodated. Please request an English certificate of vaccination from the vaccine provisional application form and send it separately by fax after clearly indicating the part to be described.
  • ● It may take several days to issue the certificate, so please leave some leeway in your schedule when requesting one.
  • ● A cancellation fee may be charged for no-shows. If you need to change your appointment, please call us as soon as possible.
  • ● We cannot vaccinate children of junior high school age or younger because we do not have pediatricians on our staff.
  • ● A guardian must accompany high school students and other minors (under the age of 20) who are receiving vaccinations.
  • ● Since this is not a yellow fever vaccination center, the yellow fever certificate (yellow card) is not available.

About the medical questionnaire form

You need to fill out the medical questionnaire form at the time of the vaccination. Please download the medical questionnaire form below, fill out the form, and bring it with you. This will make for a smooth process on the day of the vaccination. (Depending on the situation on the day of your visit, some waiting time may arise.) The consent of a parent or guardian is required for vaccinating minors. Minors presenting for vaccination unaccompanied by a guardian must bring a medical questionnaire form downloaded in advance and signed by their guardian. The minor will then be able to receive the vaccination if the doctor can connect with the guardian over the internet at the time of the history taking. *Please be advised that vaccination may be cancelled at the doctor’s discretion.

We can enter the vaccines given at our clinic into the WHO (World Health Organization) record book. 3,300 yen (tax included) per volume. *Please note that this is not the same as the yellow card for yellow fever vaccination.
